Preparing for Camp

Medical Records:

All medical records must be completed and be on file at Summer Camp by June 1. This includes an to-date records of a doctor’s physical exam and immunizations. 

Medical records are required. Your child will not be admitted to camp on the first day unless we have a completed medical record  on file.

Records may be emailed to or dropped at the church office M-F 9am-2:30pm.

We recommend you START EARLY with this process. Specific directions are included in the medical questionnaire in the camp application form.

You must provide medical records as doctors and other health professionals will  release medical records to a parent or guardian only.

Recommended supplies:

What does a child need to bring to camp?

  • suitable play clothes

  • sturdy shoes for outdoor activities

  • extra set of clothes as needed

Meals:

Note that breakfast, lunch and snack are provided by the Poughkeepsie City Schools Summer Feeding Program.
